Service Learning Reflection

I chose to go to my home town to work with Kid's Klub for my service learning project. Kid's Klub is a ministry of Wesley United Methodist Church offering quality before and after school care and summer camp programs for children ages five to fourteen. The reason I chose to go to my home town for this project is because I regularly participate in this program when I am home for breaks. While I am there I assist the children in any area in which they may need extra help, as well as helping with preparations that need to be made for activities and field trips. This project relates to the passage of Matthew 25:31-40. This passage talks about children in reference to helping each other out when one is in need and doing the right thing. Another passage that I would like to mention is Proverbs 22:6. This states that a child should be raised in the way he should go, and even when he is old he will not depart from it. Kid's Klub is a program that offers care to children before and after school. Being that it acts similarly to a daycare it gives children the opportunity to work on homework or projects they might need to finish, or just to have time to interact with other children and enjoy games and activities. In addition, they are taught scripture in fun ways that they will enjoy. Being able to do such activities in this environment gives the children numerous opportunities to help one another. It is also great that the children are able to associate with different age groups; they are always able to teach each other new things and be able to ask advice from someone that they might feel more comfortable talking to since they are closer to their own age. While I was there, I acted as a teacher, or mentor, as well as a friend. It was my job to assist them with their homework, projects and to clarify what they were going over in bible classes. I also helped prepare dinner for them and played games with them until their parents were able to pick them up. I was also there to mediate and help the kids make the right decision when there were conflicts or arguments. By doing so, they are learning how they should deal with situations and what is the right or wrong way to handle them. It is really amazing to see this first hand when I see the kids oluntarily helping one another with assignments and resolving issues on their own in ways that benefit everyone involved positively. The probability that one might help another is much greater when they have been shown the same sort of assistance when they were in need. Once they are shown this, it will stick with them through their lives and they will always know what is right and wrong no matter what. It is not as simple to teach what is right and wrong in this day and age, which is another reason why it is so important. Because of today's society, it has become a standard to disregard morals that we should learn as children and keep with us through our lives. â€Å"In today’s tumultuous society, morality often gets brushed aside as just another part of the fairy tale, and those learning moments of our youth become just another ‘happily ever after' tale that seems unbelievable by today’s standards† (Lebeau). I think it is equally important to teach values so that children are sure to be able to employ them in their lives by helping others. The main passage that I chose is Matthew 25:31-41. This passage talks about treating one another right. Ultimately how we treat one another will determine whether we are saved. It's purpose is to decide whether or not one will be permitted to the eternal kingdom or will be consigned to eternal punishment (NIV, 25:41). The King will separate the children according to whether or not they have helped others when they were in need. This, in turn, makes the obvious point that we need to be taught and to teach others hat is right or wrong and that we need to help each other, regardless of who we are; we do not discriminate. This is shown in one of the verses as follows: â€Å"The King will reply, ‘I tell you the truth, whatever you did for one of the least of these brothers of mine, you did for me'† (25:40). When we do good to others, we are doing good to god. Even though we do not have to opportunity to show this directly, it is seen and is important. Both of these passages are of equal importance when addressing what is right and helping others. Children are the future of humanity, therefore it is essential that this is stressed during the early years of their lives. These situations are presented everyday in life, young and old, and it remains important through all of our years. That is why I enjoyed working with the kids and the fact that I was an influence on them. There is nothing I love more than helping children understand why we are to do what is expected. This was certainly a beneficial experience for me as well as the children and we can all take valuable information and learn from our time together.

Whether the patient wants to complete a Do Not Attempt Resuscitation (DNAR) form or not: Healthcare professionals have an important role in helping patients to participate in making appropriate plans for their future care in a sensitive but realistic manner, making clear whether or not attempted CPR could be successful. Helping patients to reach a clear decision about their wishes in respect of CPR should be regarded as a marker of good practice in any healthcare setting (British Medical Association (BMA), the Resuscitation Council (UK) and the Royal College of Nursing (RCN) : 2007)4 And finally, what right the patient has to be involved in decisions about their medication. Patient and family partnerships are very important in the care decision process because without them numerous problems may arise. Some of which are: self-discharge, readmission, deterioration of condition and death. (Smith and Hider: 2009)6 According to Gott et al. (2000)7 successful patient partnerships are those in which the patient and healthcare practitioner meet as equals with different expertise. The doctor or nurse has the medical knowledge and skill, but the patient has personal knowledge and skill. This is useful because it provides two different perspectives which in turn will provide the best outcome for both the patient and the healthcare provider. For the most part, patient and family involvement is beneficial to the care process because the patient knows their own condition the best and if we assume that they are close to their family and share their struggle with managing their condition/disease with them then they too know what the patients condition well. Anderson and Funnell (2009)8 state that patient and family involvement in care decisions revolve mostly around decision making and empowerment. Patient involvement in decision-making is now generally regarded as a feature of good quality health care. Many health professionals, institutes and government policies such as the Department of Health (2007)9 now advocate that patients should be involved in some way in decisions about their health care. According to the RCN (2003)10 empowering patients is a central element of nursing care, by forming a solid patient- nurse partnership, and encouraging the patient and their family to be part of the decision making process, this allows the patient to feel empowered and in control of various aspects concerning their health. However according to a study carried out by Dickerson (2004)11 shows that although patient and family involvement is crucial in care decisions, various authors might argue that a lot of patients search for and find information/new treatments about their illness/ condition on non-credible sources such as the internet, magazines and social media and believe that whichever fact or new never trialled before treatment they read or hear about might be a diagnosis to their symptoms or a solution to their illness. The study showed that many patients (50%) relied on friends and family to navigate the Web, and most of patients reported that the information that they sought was unrelated to their clinical visit. According to McMullan (2005)13 Health professionals are reacting to the more Internet informed patient in some of the following ways: The health professional either feels threatened by the information the patient brings and responds defensively by asserting their expert opinion (health professional-centred relationship). Or, the health professional and patient collaborate in obtaining and analysing the information (patient-centred relationship) (Pautler et al. : 2001)14. Although the health professional almost always comes to a decision to progress with the latter option, feeling threatened and being defensive about the internet based information being presented to them by the patient and their family is usually the health professionals first reaction. An alternative approach would be for the health professional will guide patients to r eliable health information websites. It is vital that health professionals acknowledge patients search for knowledge, that they discuss the information obtained by patients and guide them to reliable and accurate health websites. It is suggested that courses, such as patient informatics are incorporated in health professionals education (Sommerhalder et al. : 2009)15. The Department of Health (2003)16 states an effective discharge as A process and not an isolated event. It has to be planned for the earliest opportunity across primary, hospital and social services, ensuring that individuals and their carers understand and are able to contribute to care planning decisions as appropriate. Here we see the DOH (2003) describing an effective discharge as one that co-ordinates all of the services needed by the patient in order for the patient to have input on the discharge and for everything to be ready for the patients discharge. Nurses and other health care professionals recognise that planning for patients hospital discharge during the inpatient stay sets the stage for effective and therefore successful self-care management at home. (Nosbusch et al.:2010)17. According to the Wales NHS effective discharge policy document (:2009)18 an effective discharge constitutes of the following 6 principles: Communication when it comes to the transfer of care process, it is important that good communication consists of mutual understanding and having a common language between everyone involved. This requires effective dialogue and sharing of up to date information amongst patients, carers, providers and commissioners. Casey and Wallis (2011)19 state that Nurses and the nursing staff are at the core of the communication process: they assess, record and report on treatment and care and handle information sensitively and confidentially .To establish a healthy nurse- patient relationship, good communication is crucial. As a nurse, building a close rapport with your patient is one of the ways to make your patient feel listened to, understood and involved in their care. Good communication is vital in the process of decision making. Jonsdottir et al. (2004) 20 state that communication skills are one of the most imperative aspects of nursing, considering that nursing always is two-folded with b oth task-oriented and relational aspects. A nurses communication skill is an essential requirement for patient participation in decision making. Communication discrepancies have been recognised as one of the major barriers to partnership building between nurses and patients (Keatinge et al. 2002)21. Good communication in the care decision process is vital In order to achieve a seamless discharge for the nurse, patient, the patients family and all other healthcare practitioners/Multidisciplinary team (MDT) members involved. The NHS Trust Discharge policy (2010)22 states that before planning a discharge, the nurse and other healthcare practitioners must decide and inform the patient and their family on whether it is a simple discharge: one that involves minimal disruption to the patients activities of daily living, does not prevent or hamper a return to their usual place of residence and will not require a substantial change in support offered to the patient or their carer in the comm unity. Or whether it is a Complex Discharge: A discharge process that deviates from the simple discharge pathway and requires complex coordination of services to enable safe discharge. To ensure that the patient and their family is involved in the care decision process of discharge, the nurse and other clinicians must be certain that an effective and well-timed discharge plan is put in place. The main objectives of this will be to plan, inform, liaison and negotiate to ensure a smooth discharge for patients and their families. Supporting this is the need for an early establishment of what the discharge dates might be, including pre-admission planning, effective communication between individuals and across settings, good clinical management plans and the alignment of services to ensure continuity of care(DOH :2010)23. Assuming that it is a simple discharge, once the discharge plan has been put into place and is carefully explained to the patient and their family, the patient can then begin to contribute in the decision making process alongside the nurse and members of the MDT team.( Shepperd et al. :2010)24. Some of the ways a patient can contribute in the decision making process when it comes to discharge is to work alongside the nurse and members of the MDT team in order to inform and help them assess whether the patient can- Obtain and self-administer medications- the patient should inform the nurse and other clinicians (such as pharmacist) on what regular medication they take, what form they prefer their medication: liquidised, dosette boxes etc. How well the patient performs self-care activities, and does the patient eat an appropriate diet or otherwise manage nutritional needs and whether the patient is able to attend any follow-up outpatient appointments (Bull and Roberts: 2001)25. Atwal (:2002)26 states that ensuring that the patient and their family have full involvement in making the above decisions and steps to be discharged from the hospital will gi ve the patient a sense of control and therefore empowerment and in turn guarantee an organized discharge as well as patient satisfaction and the nurse feeling confident that they have done their best for their patient and therefore feel fulfilled in their role as a nurse. It is prominent that a patient must be an active participant in his/her empowerment, signifying that Nurses cannot empower patients: the role is to facilitate and support the empowerment (Laverack :2005)27. .

Impact of Harassment on the Victim How does harassment have a psychological effect on homosexuals? Homosexuality has been an issue for people around the world seemingly since the beginning of time. It is a lifestyle that has been debated, unaccepted and stereotyped.But in recent years, society has welcomed more and more homosexuals into its loving arms under the umbrella of civil liberties. It is become increasingly acceptable to be homosexual in society today.1 Investigation made by the National Gay and Lesbian Task Force Policy Institute including some surveys, found that there was a high percentage of discrimination against homosexual people. The investigation also stated that homosexual people have reported discrimination in public restaurants, medical facilities, insurance, and also education.2 Due to the investigation results the civil rights bills have introduced into the process that will give particular rights of nondiscrimination based on sexual orientation. Now a day as the adult homosexual community increases day by day, there is also an increasing number of homosexual youth in the present day. Results shown by the 1996 National Longitudinal Study of Adolescent Health found that in over 12,000 teenagers, ages between the 13 and 18, six percent reported same-sex attraction.3 So how does harassment influence a young homosexual psychologically? In order to answer the previous question, we must first deal with issues concerning homosexual discrimination while evaluating facts on the harassment and discrimination that homosexual teenagers have been exposed to and the psychological outcome that this has had on them. Also we must define what sexual orientation is, and study information of how many people in America are homosexual. We must then use the biological perspective to explain the origin of homosexuality. Over the past two decades data supports that there has been discrimination among the homosexual teenagers and that they are regularly being harassed by other people. The different types of harassment that the homosexual people undergo on a dail y basis are presented later on in order to discuss the suffering and psychological problems. Along with the explanation of the different types of harassment, studies made present results that suggest that the victims of the harassment had some type of psychological problem. The Psychology behind Homosexuality: In general, a variety of biological and behavioral traits of homosexual people are different from those of heterosexual people. There are biological associations of sexual orientations found among brain differences, genetic influences, and prenatal hormonal influences. Now a day investigations propose that sexual orientation is, at least, somehow biological. The British-American neuroscientist, Simon LeVay, discovered while studying dead homosexual and heterosexual people, that some sections of the hypothalamus have a connection with homosexual identity. He established out that hypothalamus cell clusters are larger in straight men than in women and gay men. He also stated that the brain does change with sexual orientation, which is completely supported by the theory that establishes that everything that is psychological is also biological.5 LeVay doesnt see the brain as a sexual orientation organ, but, as an essential part of the neural trail connected in sexual behavior. Scientists today suppose that homosexual orientation is influenced by genetics, environment, and has biological tendency. In March of 2005, UCLA researchers reported the first study that examined everyday school harassment, which was conducted on the 192 sixth grade students. 47 percent of them reported being bullied at least once during the development of the study, while 46 percent said that they had been bullied at least once. This study is in some way helpful because it addresses the psychological problems that these students were left with after being harassed. Juvonen, Mark and Graham Schuster wrote, Students who reported getting picked on also reported increased embarrassment and fury, while students who saw a classmate picked on reported increased concern and disliked school more.12 Adrienne Nishina, Jana Juvonen, and UCLA developmental psychology graduate student Melissa Witwok recently published in the Journal of Clinical Child and Adolescent Psychology, Middle school students who are bullied in school are likely to feel depressed, lonely and miserable, which in turn makes them more susceptibl e to further bullying incidents12 Because they are different from their school mates, the bullying that homosexual students are subjected to, is considered out of the normal. Homophobia is one of the main reasons the teenagers are often harassed, and homophobia starts early and is often influenced by the adult figures in the childs life. The Institute of Education conducted interviews in 1987, with teachers at schools, and it concluded that 82 percent of teachers are conscious of homophobic bullying, although the school still lacks the attempt to deal with these issues. Homophobic bullying and harassment has been estimated to be the reason of one in five homosexuals attempt to commit suicide or harm themselves. Bullying and harassment are types of disturbance and, therefore, may lead to cause damage to ones health. The warning signs that the victims usually present are the ones that are identified as Post Traumatic Stress Disorder. Aggression and harassment frequently cause the PTSD to be more difficult. Complex PTSD is often untreated because the sufferers of PTSD tend to find it difficult to find any kind of treatment or knowledge of treatment, and, if untreated, PTSD symptoms can last for a lifetime. Suicide, a Result from Harassment: Gay and lesbian teenagers have increased rates of assault, suicide, substance abuse, and homelessness; these events are a reflection of what homophobic attitudes express by others or may be internalized feelings of self-hatred. Adolescents who have by now decide that they are gay or lesbian can happen to be depressed or act out, and commonly become absent or even run away from home, or they may get to express negative feelings towards the family members. Several state and national studies have revealed that, among homosexual teenagers, there are more thoughts and actual suicide attempts than among heterosexual teenagers. Most of the time psychologists consider suicide as a way to call the attention of people. They also believe that reasons for suicide are not only external but also internal, meaning there is a chemical imbalance. They have not found any relations of suicide of homosexuals due to internal problems. This is important because it strongly suggests that sexual harassment is the major cause of suicide of homosexuals with internal problems. Statistics for homosexual suicide have fluctuated between 30 percent to less than 1 p ercent. Peter LaBarbera, in an article titled The Gay Youth Suicide Myth, uses statistics provided by Dr. David Shaffer, one of Americas authorities on suicides throughout todays teens. LaBarbera quotes Dr. Shaffer by saying, by broadening the definition, we therefore ran the risk of overdetermining the number of gay suicides. In spite of this, out of 107 male teen suicides, only three were known to have talked to others about a homosexual experience (2.7%) and two of these three died together in a suicide pact. A further four showed some behavior that could have been indicative of homosexuality. None of the female suicides were thought to have been lesbian. If all of the teenagers who showed any evidence of homosexual behavior were indeed gay, then a total of 6% of the suicides in our study would have been gay. Once again, there is no evidence that a large proportion of suicides are gay, but [6% is] more or less [what] we would expect in the general population17 In Oklahoma suicide, is the second most common cause of death in people among the ages of 13 and 25, including both sexes and all races. In the year 2003, according to the National Center of Injury Prevention and Control, 76 young adults in Oklahoma committed suicide. Out of that, five were homosexuals. Oklahomas statistics were considered because it is believed to be an enthusiastic and conservative state, considering it was one of the 11 states in November 2004 approving [the] constitutional amendments codifying marriage as an exclusively heterosexual institution. Additionally, the state defined it as a misconduct crime to issue a marriage license to a same sex couple.18 Throughout the United States, there were 4620 suicides in the year 2003, and the homosexual community accounted for 277 for all those deaths. Taking into consideration the population of the United States, 277 does not seem like a lot of people, but considering that the homosexual community only makes up 1percent or 2 percent of the whole country, it is quite a few. Conclusion: In conclusion, harassment does have an effect on a homosexual individual by making him either feel stressed, afraid of being open with his sexual orientation, or even suicidal. One of the psychological results from homosexual harassment is PTSD, which weakens the victim from the pressure of a traumatic occurrence; bullying and sexual harassment can cause this also. The most crucial result from homosexual harassment is suicide. The suicidal teenagers feel that the pressure of the attacks is too challenging and that they cant live with the weight of being homosexual. Suicide is the result of all types of harassment, including verbal, physical, and sexual harassment, which is the reason that discussing homosexual harassment is so important. The reason that homosexuals are treated differently is because people are not well-informed about homosexuality. The National Association of School Psychologists recognizes that children who question their sexual orientation are at a greater risk of harmful situations, such as suicide, physical and verbal harassment, and physical violence. These risks usually take place for the child at school, and in their study the NASP concluded that 28% of the students missed at least one day of school as a result of feeling unsafe because they questioned their sexuality. Their suggestions for creating a safe school for sexual minority teenagers include establishing and enforcing non-discrimination policies that apply to all students, educating students and staff, intervening when a student is harassing or being harassed, along with giving that student support. In order for the homosexual youth to feel secure at school, these students need to be incorporated in the non-discrimination policy that most schools have. The students and staff then need to be involved because of all the consequences that may follow the harassment of a homosexual student or peer. The education of students and staff about sexual minority teenagers should include a meticulous in service training on all diversities of human beings, as well as including pertinent research of the risks experienced by homosexual youth. Consequences should be applied to the perpetrators, but psychotherapy should be offered to both the victim and the perpetrator.
